Medicare Update
Medicare spending has been doubling every ten years. This is unsustainable. The Center for Medicare and Medicaid Services (CMS) is looking for ways to improve quality at the same time as reducing costs. A major focus for controlling costs is reducing avoidable hospital admissions and readmissions through better care transitions.
Objectives:
• Following this presentation the participant will be able to:
• Articulate the main drivers of cost in the older adult population.
• Discuss the importance of moving from a fee-for-service system of care to one based on quality outcomes.
• Define the barriers to effective communication and quality care transitions.
• Identify programs and resources to improve care transitions in one’s own community.
This activity has been submitted to the Illinois Nurses Association Approver Unit for approval to award contact hours. Illinois Nurses Association is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center’s commission on Accreditation.
This program has been pre-approved by the Commission for Case Manager Certification to provide continuing education credit to Certified Case
Managers (CCMs).
This activity has been approved by the NASW for 1.0 credit hours for social workers.
